scroll down to the bottom of the page, and click on "Support a Fund."
A fillable form will load, on which you should designate
"The Katherine Elizabeth Foundation Fund"
as the fund to which you are donating.

 You can then either print out the completed form and mail it to the address above
or e-mail it to the e-mail address given on the form.
If you prefer, you can call Dade Community Foundation’s Development Department at (305) 371-2711
and donate by charge card over the phone.
 
If you would like specify that your donation be used to assist with funeral and related expenses parents who have lost their infant child, or with the expense of a grave marker for the child,
please state on your check’s memo line or on the form:
“For funeral and related expenses” or “For grave marker expenses.”
(If you donate by phone, please be sure to give the same information.)
